Duties:

 

As a Performance Improvement Coordinator, you will:

  • Review and monitor the following OASIS documents and Medicare Plan of Care documentation for completeness, accuracy and consistency to ensure compliance with established criteria: Start of Care and/or Resumption of Care, Follow-up documentation and corresponding addendum orders, and Transfer and/or discharge documentation and corresponding addendum orders
  • Schedule and participate in active record audits for compliance including accuracy and timeliness OASIS data, presence of signed physicians orders for services/supplies, utilization and quality of care.
  • Evaluate and advise team staff regarding interpretation, implementation and compliance to Agency policies, State and Federal regulations and JCAHO standards.
  • Track and summarize occurrence reports, record reviews and other performance improvement activities, including OASIS Adverse Events reports, incident reports, patient concerns or complaints, JCAHO preparation, patient satisfaction surveys and benchmarking.
  • Monitor and coordinate the implementation of corrective action plan for deficiencies identified through utilization review, clinical record audit, Adverse Event Reports Medicare claim denials, client satisfaction surveys, sentinel event review, JCAHO and/or State surveys, and the Quality Improvement Plan.

Qualifications:

 

Qualifications:

  • In-depth understanding of Medicare Conditions of Participation, documentation and case management; strong interpersonal skills; ability to communicate effectively in writing and verbally with people who have varied backgrounds.
  • License and current registration to practice as a registered nurse in the state of agency or other compact state
  • Baccalaureate Degree in Nursing, or equivalent experience in acute and home health care setting, required.  Masters Preferred.
  • Two (2) years experience in community health nursing setting with five plus (5+) years of progressively responsible nursing experience, supervisory experience preferred.
