We are looking for an enthusiastic, self-directed individual to join our team as a Home Care Coordinator!

Responsible for ensuring continuity between the hospital and the home setting in order to maximize patient satisfaction of home health services.  This position is a sales/service position, handling new HH referrals at Wellmont - Bristol Regional Medical Center & local physician offices. 

Qualifications:

Must have one year of healthcare, sales or nursing case management.  Clinician required, RN preferred.
