Qualifications:

Qualifications for position of Central Plan of Care Coordinator:

·                    Registered Professional Nurse, a graduate of an approved school of professional nursing who is currently licensed as a registered nurse in the state of agency operation.

·                    Shall have at least two year experience as an RN with two years experience in home care.

·                    Experience in Coding and preferably certified in ICD-9 Coding.

·                    The nurse shall be available at all times during operating hours.

·                    Prefer supervisory and administrative experience.

·                    Conducts reviews of OASIS for MOO items inconsistencies, functional severity requiring therapy, and codes according to ICD-9 CM Coding standards.

·                    Maintains correct and professional communications with the public, patients, employees and other agencies, reflecting the mission of the Company.

·                    Gives direct and understandable response to questions.

·                    Articulates in a wide range of difficult communication situations.

·                    Speaks clearly and persuasively.
